The Current “State of Healthcare” in Oregon
The intersection of rising healthcare cost, and a massive loss in federal funds, poses an existential threat to Oregon: a threat to access, a threat to the stability of our general fund and a threat to our economy. Embedded in this crisis, however, is a profound opportunity to reimagine and redesign our current healthcare system, and to transform it into one that not only ensures timely access to affordable, quality medical care for all Oregonians, but is also economically sustainable and financially stable.
Developing a Shared Vision for Health Care in Oregon
Since 2025, TFME has served as a “safe forum” and neutral convener for the Health System Sustainably Group (HSSG) — leaders from hospitals, physician organizations, and insurers — which has been developing recommendations for the Governor and the Legislature to stabilize Oregon healthcare system, and chart a safe course forward through the difficult waters that lie ahead.
Because the crisis in cost and access was decades in the making, it cannot be turned around in the course of a single biennium. Therefore, the HSSG has proposed a six-year, three-biennial strategy, guided by a long-term policy vision for what we want Oregon’s health care system to look like in 2033. This “North Star” Vision was adopted by the 2026 legislature.
To realize that vision, the HSSG delivered a comprehensive three-part set of recommendations to Governor Kotek on July 10, 2026.
These recommendations include:
